Welcome to Down to Earth Counseling: counseling for teens, young adults, partners, and families, along with workshop facilitation and training.

About Down to Earth Counseling:
Adam Tannenbaum, LCSWA (they/them pronouns) is a counselor and facilitator based in Durham, NC. They have been involved in developing and facilitating dialogues/workshops to process social identities (such as race, class, and gender) for nearly 10 years. In addition to providing professional trainings, Adam offers therapeutic counseling services. Having previously interned at the Duke Child and Adolescent Gender Care Clinic, they are especially interested in working with teens, partners, and families to explore how their identities & experiences affect who they are as individuals and their relationships with other people.
Adam most values building trusting relationships with their clients. They strive to offer a nonjudgemental listening space and believe that clients have the right, and the ability, to make the best decisions for themselves. In general, Adam is process-oriented and often focuses on how different dynamics impact clients’ life experiences.

Counseling Services:
Down to Earth Counseling offers online counseling including, individual counseling (ages 12+), partners/couples counseling (including polyamorous partners), family counseling, letters of support for gender transitions, and group counseling. Primary focus areas are exploring and supporting identity development (racial identities, LGBTQ+ identities, etc), developing healthy relationships, processing trauma, improving communication, understanding family dynamics, resolving conflict, and navigating life changes/everyday stressors.

Counseling Service Cost:
My standard rate for counseling is $120 per hour. I also offer some sliding scale spots starting at $60 per hour. Please contact me to discuss sliding scale availability. At this time, I am in network with Blue Cross Blue Shield PPO, Indemnity, Blue Care HMO, and State Health Plans. I am out of network with ACA marketplace plans, such as Blue Value, and all other insurance plans. If I am out of network for your plan, your insurance company may reimburse you for the partial cost of a session, depending on your out of network benefits. I can provide you with billing information you need to receive reimbursement but please contact your insurance provider to confirm your benefits and for more information. Note that you can cannot file for insurance reimbursement if you are also a sliding scale client. For clients who are only seeking letters of support and not ongoing therapy, as part of the Gender Affirming Letter Access Project, I have pledged to offer at least one free letter of support session per month. How To Get Started with Counseling:
First email [email protected] to request access to Client Portal and intake forms. Then complete New Client Intake, Demographic Info, HIPAA Notice of Privacy Practices, and (if applicable) Additional Partners or Family Intake forms via Client Portal. Alternatively, I can email the intake forms as word documents or we can complete them over the phone. Please contact me at [email protected] or  919-443-5018 for these alternate options. After the intake forms are complete, I will reach out to schedule a free 30 minute phonecall to answer questions, confirm logistics/expectations, and (if needed) clarify information from intake forms. If we are already completing the intake by phone, then we will likely discuss these things during the intake phonecall. After we have confirmed logistics/expectations, we will schedule our first appointment together. Note that if we complete forms by phone, I will likely need a signed document or other legal notice from you to confirm that we have completed those forms verbally. 

Group information:
Down to Earth Counseling currently offers two groups. The first is a process group for transgender and nonbinary teens. It will be meeting on Tuesdays 4:30 to 6pm via Google Meet and will run from November 10 to December 29. The group is intended as a space for transgender and nonbinary 14 to 16 year olds to connect with others who are going through similar experiences and process things as they come up. Example discussion topics include exploring gender expression, dealing with dysphoria, coming out experiences, and relationships/dating. 

The second group is a process group for guardians of transgender and nonbinary teens. It will meet on Wednesdays from 12 to 1:30pm from November 11 to December 30. Some example discussion topics include understanding teens’ gender identities (and how to relate to them), how to support teens in coming out to extended family, helping teens explore their gender expression, and discussing concerns about the future. Note that the guardian group is not limited to guardians of teens participating in the teen group. It is open to any North Carolina guardian of a transgender/nonbinary teen age 13 to 19.

The group fees are $45 per session. There are some sliding scale spots available starting at $35 per session but please contact for availability.

How to Register for Groups:
First email [email protected] to indicate which group you are interested in and request access to Client Portal and intake forms. If registering for teen processing group, have teen complete Intake for Teen Group and Demographic Info. Have a guardian complete Guardian Supplement for Teen Group and HIPAA Notice of Privacy Practices. If registering for guardian processing group, complete Intake for Guardian Group, Demographic Info, and HIPAA Notice of Privacy Practices. After receiving the appropriate forms, Adam will reach out to schedule a free 30 minute phonecall to answer questions, confirm logistics/expectations, arrange a payment method, and (if needed) clarify information from intake forms. After the phonecall, upload copies of ID and submit the Consent to Treatment form, also via Client Portal, before attending the first group session.

Workshop Facilitation and Training:
Having trained in a variety of models—including Theater of the Oppressed, Healing and Rebuilding Our Communities (HROC), Critical Conversations, and Intergroup Relations—Adam offers extensive experience developing and facilitating interactive workshops on numerous topics. Recent workshops have included training emergency department nurses on working with transgender and nonbinary patients, co-facilitating “Relationships Matter: Determining the Needs of Your Transgender and Nonbinary Patients” at the Gender Spectrum Professionals’ Symposium, and co-facilitating “How Whiteness Shows Up In Clinical Practice” for a multidisciplinary consultation group.
