Standard rate: $150/hour

Initial intake: $180 

*Letter of support: $1-$180 for 1.5 hour session

At this time, I am in network with Blue Cross Blue Shield PPO, Indemnity, Blue Care HMO, and State Health Plans. I am out of network with ACA marketplace plans, such as Blue Value, and all other insurance plans. If I am out of network for your plan, your insurance company may reimburse you for the partial cost of a session, depending on your out of network benefits. I can provide you with billing information you need to receive reimbursement but please contact your insurance provider to confirm your benefits and for more information.

2. Fill out the appropriate intake forms, including signing the HIPAA Notice of Privacy Practices, and (if applicable) Good Faith Estimate

3. Adam will reach out to schedule a free 30 minute phonecall to answer questions, confirm logistics/expectations, and (if needed) clarify information from intake forms.
